Care of elderly patients .
- Elderly patients are those patients who are over 65 years of age. With advancement in health science, elder people have a chance of living longer. Since most of the elderly people seek dental care in the form of oral rehabilitation. As we age, there are variations in the physiological parameters. There is decline in our physiological function. Sometimes ageing predisposes to pathological changes as well. The ability to adapt to stressful conditions also reduces. Usually at this age patients may be suffering from one or the other conditions such as diabetes, cardiac disease. As they are already suffering from an ailment the patient should discuss with the dentist before undergoing dental treatment. There are conditions that may influence the dental care. The patient should tell about the condition he is suffering from, medication which he is taking . As the drugs prescribed by dental professional should not interact and produce any effects. There are newer methods for oral rehabilitation such as dental implants which may provide the patient with better results as compared to removable prosthesis.
- 2 . Communication between patient and doctor should be there so as to have good relationship with each other. Each patient which goes for treatment should provide the doctor with information of medical history. This can be in a form of verbal /nonverbal exchange of information. This verbal communication will help to reduce or eliminate incidence of fear, anxiety a nd phobia. Fear is defensive response to actual threat. Fear in the dental clinic may be for pain, needle, extraction, surgery, sight of blood , sight of instruments etc. If there is good rapport with the doctor then patients are less fearful during the procedure and therefore less chances of anxiety related complications.#carepatients #Dental #Tourism #32teeths.com #healthcare #dentalchain#dentalImplant #bottox #toothextraction #toothPain

Bad Breath - Many patients suffer from bad breath. The patients who realise that they have halitosis, do not feel comfortable when speaking to someone. There are various reasons for bad odour from mouth. It could be related to the type of food which we eat, such as food with intense odour e.g onion, garlic. Sometimes it could be due to alcohol, smoking, tobacco. Other reasons are poor oral hygiene, gingivitis, periodontitis, sinusitis & throat infection, patients with diseases such as diabetes, keto acidosis condition, liver disease, kidney disease, lung infection, dehydrated patients and dry mouth( xerostomia) .Dry mouth is usually seen in patients who are mouth breathers, on medications, salivary gland diseases, patients who have undergone chemotherapy, radiotherapy.
It is advised that patients should seek an appointment with dental professional to find the causative factor and get rid of bad odour .
It is advised that after your meal you should do brushing and flossing of your teeth. Use of mouth wash can also help in reducing odour from mouth. As food is lodged in between teeth there is increase in the bacterial count, release of toxins which can lead to bad odour.
Always brush two times daily.
Tongue surface should also be kept clean by brushing. Use floss, interdental brush to remove food lodged between the teeth. Those who wear dentures, they should remove the denture at night and denture should be cleaned with brush regularly.

1. Awareness: The Indian lifestyle is not very congenial to the oral health. The common man has been historically using tobacco & alcohol which is detrimental to dental health. In rural areas people adopt archaic oral hygiene practices which aggravate the problem. Due to costly dental treatment, they end up losing either teeth or life.
2. Approx 60-80% of our children suffer from dental issues and periodontal (gum) disease affect nearly 90-95% of our population. Lack of adequate & affordable dental care affects almost every individual in India.
3. Late detection of disease: Due to habit of procrastination & lack of awareness of dental problems like oral cancer & gum diseases, it becomes difficult to cure them at late detection which increases the cost of treatment & is a threat to life. Regular dental check up is an issue in India.
4. Costly technology: The dental technologies in field of implants, RCT, Ortho etc makes its implementation difficult for common masses. The increase in the number of treatments, cost effective technologies, efficiency in operation etc will bring the economies of scale & scope, which will result in democratization of dental services consumption to the bottom of pyramid.
5. Prevention is better than cure: There is a requirement to develop a holistic model to address the preventive needs as well as the disease specific requirements, increasing awareness, initiating frequent check-ups & operational & supply chain efficient model to overcome the prominent issues in providing quality, affordable & adequate dental services to masses.
Right from small changes to major repairs, dentist can perform different procedures to improve smile. The procedure can be used if the teeth are discoloured, chipped, misshapen or missing. Once the procedure is done, patient gets are perfect smile. Some of the common procedures are bleaching, bonding, crowns, veneers & contouring and reshaping.
A dental crown is a tooth-shaped cap which is placed over a tooth to cover it & to restore its shape and size, strength, and improve its appearance. When the crown is cemented into place it fully covers the entire visible portion of a tooth that lies at and above the gum line. It’s used to protect a weak tooth from breaking or to hold together parts of a cracked tooth, to restore broken tooth or a severely worn down tooth, to cover and support a tooth with a large filling, to hold a dental bridge in place, to cover misshaped or severely discoloured teeth, to cover a dental implant & to make a cosmetic modification. Various options of crowns are Stainless steel/metal crowns, porcelain-fused-to-metal dental crowns, All-resin dental crowns, all-ceramic or all-porcelain dental crowns & Zirconia or milled crown.
Dental X-rays emit low amounts of radiation and precaution is taken to protect patients from exposure of these radiations. Apart from this some other issues of Dental X-rays are the wait time for film development and environmental concerns. Digital radiography addresses some of these issues. This has advantage over conventional radiography as it reduces exposure time to approx 90%, reduces waiting time, high quality digital image that can be transferred to computer & above all its environment friendly.
You should brush your teeth two times daily. You should spend almost 3 minutes for oral care. It should be done with soft bristled brush. Brushing should be done gently. The strokes of brushing should cover inner, outer and chewing surfaces of all teeth. With tooth brushing we remove the dental plaque/tartar from tooth surfaces which will prevent caries/cavities, gum diseases, gingivitis.
It is recommended that the patient should visit dentists for a checkup after six months. If plaque and calculus /tartar is present then professional cleaning which is scaling, tooth polishing followed by fluoride treatment should be done .after this cleaning oral hygiene is essential for preventing tartar deposits or build up. So tooth brushing combined with dental floss to prevent accumulation of plaque on the teeth.
Inter dental brushes/proxy brush is a small brush used to clean in between the teeth. it can also be used to clean between the wires of dental braces and orthodontic wires.

It is an important method of maintaining oral hygiene. it removes plaque and food debris which is stuck in between the teeth. The food which gets lodged in between teeth gets decayed and causes increase in bacteria which causes damage to the gums. There will be bleeding from gums, bad smell/halitosis.
It is advised that Flossing should be done once per day before brushing. The floss length of 10 inches is taken and 3-4 inches is put in between the teeth. It is then wrapped in around the middle finger & or index finger & supported with the thumb. Then it is held tightly to make taut &it is gently moved up and down.

Cancer involving the oral cavity is a concern to the health as this is a cause of morbidity and mortality. The oral cancer can involve the lips, cheek mucosa, bone, tongue, soft tissue below the tongue.Oral cancer can occur because of the following factors.
Chewing of tobacco, supari &paan ,bidi smoking and reverse smoking.Alcohol is a solvent which allows carcinogens to enter in the mucosa. Another etiology of oral cancer is human papilloma virus.Other factors which may contribute are chronic irritation with sharp teeth &crown, denture which are ill fitting .
There are various precancer lesions and conditions which may often progress to oral cancer. It is very important to identify these conditions at early stage so that patient is routinely checked for any progression of such lesions or their transformation into malignancy.

The patients may face various problems due to absence of teeth such as lack of function i.e. loss of chewing function and difficulty in eating. Individuals with missing teeth and loose dentures make people shy & they try to avoid public places. Tooth loss can be due to sports injury, excessive wear or grossly carious tooth or periodontal problems.
It has been seen that if jaw bone remains without teeth, it will resorb /disappear because our jaw bone needs chewing action of teeth to stimulate and keep it strong. As the time passes the patient who is without the support of teeth & bone, the face will begin to look aged.
Treatment options for replacing teeth –They can be replaced by use of crown & bridge, partial denture or full dentures.
Crown & bridge require two or more healthy teeth to serve as abutment. They are reduced in size before cementation of the bridge & hence increase the chances of cavities & endodontic treatment.Another option to replace teeth is dental implant.A dental implant is a root like artificial structure made up of titanium which is placed in the bone . After a period of osseointegration which is around 3-6 months an abutment is attached to it and over it crown is either cemented or screwed.Dental implant can replace single tooth or several teeth.
They are very conservative form of treatment as the adjacent teeth are preserved. They also help in maintaining bone structure . Dental implant like teeth transmit chewing forces to jaw bone and maintaining bone health.Dental implants are the most preferred form of treatment for missing teeth .They have high success rate (90-95%).
Implant are available in different designs, different sizes .Dental implant can replace single missing tooth provided there is adequate bone at the site. If adequate bone is not available then bone grafting can be done. Multiple teeth replacement can also be done by Implant supported prosthesis (removable/fixed), or implant supported denture. Some patient who have lower denture which is very loose, they can benefit from implant supported over-denture.

MPDS (myofascial pain dysfunction syndrome) is a disorder in which the patient suffers from pain & discomfort in the Temporomandibular joint area , muscles of head, neck & shoulders, and in or around the ear when the patient chews, speaks, or while opening mouth.
Usually the pain is continuous and dull in nature.Sometimes there are trigger -tender points (pain when you press/palpate).There are various factors which cause MPDS.Some of the causative factors include stress, sleep disturbances, injury , mal occlusion ,patients with parafunctional habits like grinding of teeth either day time or while sleeping.
During emotional stress because of hyper functioning of the muscles there is muscle fatigue.Teeth clenching, grinding of teeth ,habit of chewing tobacco or chewing gum can also cause muscle spasm.
Occlusal problems such as faulty restorations, missing teeth for long period can cause occlusal imbalance and changes in vertical dimension.Occlusal disharmony can cause muscle spasm and may lead to pain.
The management of MPDS includes counselling and assurance to the patient.Pharmacotherapy include-analgesic with muscle relaxant, antidepressants. Oral appliance-occlusal splints, night guard can be provided to patients who have a habit of grinding teeth at night. Identification and correction of causative factor should be done . The causative factor are corrected such as replacement of missing teeth with crown ,bridge or denture and orthodontic treatment for correction of malaligned teeth .There are few home measures which patient can practice at home such as use of moist heat or cold packs for local application for about 10 minutes. Patient is advised to eat soft diet.Extreme jaw movements are avoided.Other treatment options include TENS( Transcutaneous electrical nerve stimulation),low level laser therapy LLLT.

Dental caries, also known as tooth decay, cavities, or caries, is a breakdown of teeth due to activities of bacteria.
Different types of bacteria are present in our mouths and build up a sticky film called dental plaque.
Brushing and inter-dental cleaning daily will help reduce the amount of dental plaque and bacteria in your mouth.Avoid sticky and sweet foods during the day to help reduce production of tooth-damaging acids in your mouth. Chewing gum can also help decrease bacterial growth.
Once the enamel has been irreversibly damaged, your dentist will have to do filling. At times , a root canal also may be necessary in delay cases.
